Breast elastography is an advanced ultrasound technique that measures the stiffness or elasticity of breast tissue. Malignant tumours are typically harder (stiffer) than surrounding healthy tissue or benign lesions like cysts or fibroadenomas. Elastography provides a colour-coded map of tissue stiffness, helping radiologists differentiate between benign and malignant lesions with greater confidence – often reducing the need for unnecessary biopsies.

Elastography is performed alongside standard breast ultrasound, adding functional information to the anatomical images. It is particularly useful for evaluating indeterminate lesions seen on mammography or ultrasound, and for characterising masses in dense breast tissue. The test is painless, radiation‑free, and takes only a few extra minutes during your routine breast ultrasound.

Prepare

Preparation

No special preparation is needed. Wear a two‑piece outfit. You will be asked to undress from the waist up and wear a gown. The test is performed during your breast ultrasound.

Scan

Ultrasound & elastography

First, standard breast ultrasound is performed. Then, the radiologist applies slight pressure (strain) or uses shear‑wave technology to measure tissue stiffness – displayed as a colour map over the ultrasound image. The process takes a few extra minutes.

Interpret

Analysis and scoring

The radiologist analyses the elastography images, assigning a stiffness score or quantitative value (kPa). This is correlated with B‑mode findings and BI‑RADS category.

Report

Report and recommendations

A comprehensive report is prepared, integrating elastography findings. You receive the report via WhatsApp/email within 24–48 hours, along with clear guidance on next steps – whether routine follow‑up, biopsy, or specialist referral.

Indeterminate breast lesion

Palpable lump with uncertain ultrasound

If a lump is felt but ultrasound findings are equivocal, elastography can help determine if it is benign (soft) or suspicious (stiff).

Follow‑up of known lesions

If you have a known benign lesion, elastography can monitor for any increase in stiffness – a sign that may warrant biopsy.

Previous benign biopsy with persistent lesion

If a lesion was biopsied as benign but remains or grows, elastography can help decide if re‑biopsy or surveillance is appropriate.

Reducing biopsy anxiety

If you are anxious about undergoing a biopsy, elastography can provide additional reassurance when findings suggest a benign lesion.
